Date： B.4 SH/T3503－J204 Measured data in “Equipment foundation survey record” including design coordinate and elevation is locating data of foundation. Topic with “Confirmation on Result of Survey” must be signed by Construction Contractor or Supervision Contractor. B.5～B.8 Specific locating data record of each section of equipment foundation could choose special forms according to different type of equipment foundation. Including SH/T3503－J205 “Block Type Equipment Foundation Allowable Deviation Item Survey Record”, SH/T3503－J206 “Integrated Framework Type Equipment Foundation Allowable Deviation Item Survey Record”, SH/T3503－J207 “Storage Tank Foundation Allowable Deviation Item Survey Record (1),(2)” and SH/T3503－J208 “Spherical Tank Foundation Allowable Deviation Item Survey Record”

etc. These kinds of forms would be delivered to Installation Contractor (refers to Take-over Party in the form) after completed the construction of equipment foundation and satisfied the quality requirement. Topic with “Confirmation on Result of Survey” of the form must be signed and confirmed by Construction Contractor or Supervision Contractor.

B.4 SH/T3503－J204 “设备基础复测记录”中的测量数据为基础整体的定位数据，包括设计坐标和标高。“复测结果确认”栏由建设/监理单位填写。 B.5～B.8 设备基础中各部位具体定位数据的记录可根据设备基础形式的不同选用专门的表格填写，包括SH/T3503－J205“块体式设备基础允许偏差项目复测记录”、SH/T3503－J206“整体框架式设备基础允许偏差项目复测记录”、SH/T3503－J207“储罐基础允许偏差项目复测记录（一）、（二）”和SH/T3503－J208“球罐基础允许偏差项目复测记录”等。此类表格作为土建施工单位即表中“施工单位”在设备基础施工完毕并符合质量要求后，交付安装单位即表中“接收单位”的用表，表格中的“复测结果确认”栏由建设/监理单位填写并签认。
